# Contact form

A contact form is helpful in generating leads, boosting sales and conversions by understanding your customers' needs.&#x20;

* Your contact form sends all submissions to your store's sender email address. You can change your sender email address on the [**Notifications**](https://admin.shopify.com/settings/notifications) settings page of your Shopify admin.
* You can't change the subject line for emails sent through a contact form on a Shopify store.

Steps

1. In the theme editor **(Customize)**, click **Add section.**
2. Locate **Contact form.**
3. Make necessary changes.
4. **Save.**

Control the **design**, **width** and **aligment** of the form.

#### Submit button

Adjust the **button label** and **style**.&#x20;

### Block settings

You can add 4 blocks to the form: name, phone, email, message.

In each block, add the **field label, placeholder** and adjust the **field width** (50%/ 100%).

The email and message fields are required for the form to submit successfully.
